You are requested to fill this proforma and email it along with attachments to the following email address:
media.oiccfm@mofa.gov.pk
Attachments that are required with the form are:
1. Passport Size picture
2. COVID vaccination certificate
3. Equipment Details
4. Copy of CNIC/Passport
It may be noted that Media Accreditation Passes to access the venue will only be made on the basis of the proforma as shared above.
